1. VERIFICATION OF RESIDENCE:
Proof of residence needs to be presented by the parent/guardian at the time of registration. 

• ORIGINAL Florida Power & Light receipt showing name and address for the past 3 months. (NO COPIES ACCEPTED)

AND

• ORIGINAL lease agreement that has been notarized or ORIGINAL mortgage that has been notarized

Failure to submit any of the above documents will result in an automatic visit from the Miami-Dade County Public Schools' Social Worker.

2. ORIGINAL HEALTH RECORDS FORMS FOR NEW STUDENTS: 

• Florida Department of Health Student Exam (Form DH3040, including proof of tuberculin skin test)
• Florida Department of Health Certification of Immunization (Form DH 680) 
  M-DCPS TRANSFERS: Must have current documented immunization records in ISIS

3. IDENTIFICATION:

• A copy of Birth Certificate or Passport

FRAUDULENT DOCUMENTS WILL RESULT IN IMMEDIATE WITHDRAWAL TO HOME SCHOOL

ADDITIONAL DOCUMENTS (NOT REQUIRED)

• Copy of student's last report card or complete transcript. 
• Social Security number if available
